The refreshed Care plan for the dying person – Victoria and supporting resources will help you explore and document someone’s wishes, requests and preferences in the last days and hours of life.

Clinical Care Guidelines – Banksia Pall Care & GRPCC

These guidelines are a great resource as a quick reference for Palliative Care Specialist Clinicians. They cover a wide range of symptoms associated with life-limiting illness and end of life care, but should be used with discretion. *Some medications are used for indications, by routes or in
doses that are not approved by Australian licensing. This is common practice and validated internationally.

The Gippsland Regional Palliative Care Consultancy Service is also referenced throughout the document – however it may be more appropriate to refer to your local consultancy for advice or referral.
